Patient Rights & Responsibilities
Purpose
To promote the interests and well-being of the patients of Dr. Treyzon Gastroenterology.
Patient Rights
- All patients utilizing the services of Dr. Treyzon Gastroenterology shall be treated with respect, consideration and dignity.
- Patients of Dr. Treyzon Gastroenterology will be provided the appropriate privacy while being seen at our offices.
- Patient disclosures and records will be treated with confidentiality. Patients will be given the opportunity, when desired, the opportunity to approve or refuse their release.
- Patients will be given the opportunity to participate in decisions involving their health care. This will be done except when participation is contraindicated for medical reasons.
Patient Responsibilities
- The office expects that a patient will provide accurate and complete information about matters relating to his/her health history in order for the patient to receive effective medical treatment.
- A patient is responsible for reporting whether he/she clearly comprehends a contemplated course of action and what is expected of them.
- The office expects that a patient will cooperate with all personnel and ask questions if directions and/or procedures are not clearly understood.
- A patient is expected to be considerate of other patients and office personnel and to observe the no smoking policy of the office. A patient is also expected to be respectful of the property other persons and the property of the office.
- The patient is expected to help the physicians, nurses, and allied medical personnel in their efforts to care for the patient by following their instructions and medical orders.
- It is understood that a patient assumes the financial responsibility of paying for all services rendered whether through third party payors (his/her insurance company) or being personally responsible for payment for any services which are not covered by his/her insurance policies.
- It is expected that the patient will not take any drugs which have not been prescribed by his/her attending physician and administered by the office staff while in the office.
- Patients are required to follow up in the office or by telephone discussion with the doctor in order to communicate:
- changes in the patient's health or treatment of their other medical conditions
- adverse reactions to medicines or treatments such as endoscopy
- confusion about treatment recommendations
- change in address so that Dr. Treyzon can mail communications to the patient
- inability to follow up for surveillance of polyps or other gastrointestinal conditions
Dr. Treyzon always schedules a follow up appointment at the end of visits so that follow up of the patient's condition can be performed. If no follow up is created and gastroenterology consultation is no longer necesaary, it is the patient's responsibility to report any new sympptoms to Dr. Treyzon or to their primary care doctor. A referral back to Dr. Treyzon can be generated.
- Patients are rquired to read and be aware of the package insert for all pharmaceuticals (medications). This includes the risks of treatment and the possible adverse reactions. Dr. Treyzon does his best to outline the major reasonable side effects, but is unable to review all possible side effects for each medication prescribed.
- If a procedure is scheduled, Dr. Treyzon will explain the nature of the procedure, its risks, benefits, indications and alternatives. If the patient does not clearly understand these aspects, it is the patients responsibility to inquire with Dr. Treyzon. Informed consent for a procedure requires that the patient understand this. It is better to not proceed or to delay the procedure until it is clearly understood byt the patient. Procedures are rarely emergent, so please take the time to inquire about concerns of any treatment or diagnostic plan Dr. Treyzon has recommended.
